MY PERSONAL DETAILS?
Please inform the receptionist of any changes to your personal circumstances as soon as possible. This includes name, address and landline/mobile telephone numbers. If you move outside the practice area you will need to re-register with another practice. We have to restrict our patient area in order to provide a satisfactory service to our local population.
